How can I find a document that I filled out to connect to my bank? -

I filled out a qiestonaire so I could sign on to my bank account at First Bank in Granbury. I would like to print it so I can remember my pass word and how to sign in.

Just call your bank and tell them that you are new to online banking and have forgotten your User name and password. They will ask you a couple of questions to verify your identity.They may reset your password but they will tell you your User name and new Password.
